refactor(db): adopt SeaORM 2.0 entity-first migration strategy

Delete lib/db/migrations/ (7 documentation-only SQL files; never executed
per the project's "数据库就不要动了" policy). Enable `schema-sync` +
`entity-registry` features on the db crate and wire the dormant
`auto_migrate` config flag into `Db::from_config` so every startup syncs
the registered `db::*` entities against the database.

`schema_extras.sql` is the single hand-maintained SQL file kept for the
constraints entity attributes cannot express: partial/overlapping unique
indexes (issue_watch, issue_reaction, project_favorites expression),
composite non-unique indexes, polymorphic XOR CHECK constraints, and
drops of the legacy channel_members / chat_members tables. Idempotent
(IF NOT EXISTS / DO-block guards).

project_links gains the composite unique_key (project_id,
related_project_id, kind) so `sync` does not drop it on existing DBs.

Tests:
  - registry_prefix_matches_module: verifies the `"db::"` prefix matches
    by module_path.
  - entity_first_sync_converges_on_sqlite: representative cross-module
    entities sync twice idempotently with FKs inlined topologically.
